Welcome to Illiana Nurse Network scheduling application powered by NursaTrix.app (“the App”). This platform is owned and operated by Illiana Nurse, LLC. By accessing or using NursaTrix.app, you agree to be bound by these Terms of Use and this Privacy Policy. If you do not agree, please do not use the App.
2. Privacy Policy
Information We Collect
When you use NursaTrix.app, we may collect the following facility or personal information:
– Name, contact details, and facility information.
– Scheduling and booking details.
– Billing department contact information.
How We Use Your Information
Your information is used to:
– Confirm and manage shift bookings.
– Deliver necessary documents (such as the Terms of Service Agreement and the Rate & Scheduling Agreement).
– Communicate with you regarding scheduling, billing, and updates.
Information Sharing
We do not sell, trade, or share your facility or personal information with third parties, except as required by law or as necessary to provide services you have requested (e.g., sending agreements through DocuSign).
3. Terms of Use
Scheduling and Booking
– Shifts booked through NursaTrix.app will remain Pending until all required documentation is completed (including the Terms of Service Agreement, Rate & Scheduling Agreement, and billing information).
– You will receive an email with the required agreements via DocuSign for completion and electronic signature.
– Please ensure the email address you provide is accurate and that it belongs to an authorized representative who can complete and sign the agreements.
– Once completed, your booking will be confirmed.
System Reliability
NursaTrix.app is a digital platform. While we strive for uninterrupted service, technical glitches may occur. If you do not receive a confirmation email for your booking:
– For urgent needs, please call Crystal directly at (219) 800-6877.
– For all other inquiries, please email crystal@illiananurse.com.
We will respond within 24 hours.
No Minimum Commitment
Facilities are not required to commit to a minimum number of hours or shifts. Agreements remain ongoing and flexible unless otherwise arranged.
4. User Responsibilities
By using NursaTrix.app, you agree to:
– Provide accurate and complete information.
– Use the platform only for lawful purposes related to nursing coverage.
5. Limitation of Liability
Illiana Nurse, LLC and NursaTrix.app are not responsible for damages resulting from:
– Delays or interruptions caused by system errors or outages.
– Errors in information provided by facilities or users.
– Missed or delayed confirmations due to incomplete documentation.
